The transaction was led by an SGK legal team composed of Eric C. Springer, David J. Lowe, Jeremy C. Brown, and Jackie Horwat. Cosán, established in 2015, is an industry-leading healthcare organization focused on a continuum of care for older adult patients using technology-driven preventative care services including concierge home care. Cosán delivers a technology-based preventative care coordination tool suite to help physicians, healthcare organizations, and payers improve patient outcomes. The preventative services tool suite includes:
- Chronic Care Management (CCM) – to improve patient health outcomes through increased oversight, communication, and collaboration between physician appointments as well as concierge home care;
- Remote Patient Monitoring (RPM) – which uses technology that enables remote monitoring of patient’s vitals; and
- Behavioral Health Integration (BHI) – to improve outcomes through support services for individuals with mental and behavioral health conditions.
